The most precious work you did was at the end with your healing touch. He was weary by then and your neck/back rubs sent him into an immediate state of relaxation. At Van Derhoof Physical Therapy, our therapists enhance quality of life by addressing the physical, emotional and spiritual needs of clients in hospice care. We work to keep clients as comfortable as possible. Our hospice based physical therapy services include:

Mobility
Assisting patients with daily activities such as getting in and out of bed, using the bathroom, and bathing, either independently or with caregiver support. Exercises and interventions aim to maintain or improve mobility.

Education
Teaching patients, families and caregivers how to perform daily tasks safely and reduce fall risks. Also teaching families how to provide comfort care measures such as massage and positioning.

Family and Team Communication
Keeping open lines of communication with family members and care team—including doctors, nurses, and other professionals—to ensure cohesive, supportive care for the patient.

Fall Prevention
Assessing the home environment, recommending necessary changes, and teaching exercises to improve balance and stability.

Pain Management
Providing massage, stretching, manual therapy, and positioning to help patients manage pain, enhance circulation and improve mobility.

Home Modifications
Adapting the home environment to safely accommodate patient’s needs as their abilities change.
